Root Cause Analysis (RCA) identifies and analyses the hidden causes of problems and provides the tools needed to determine what can be done to prevent them from reoccurring. RCA illustrates how to drill down to the root cause of problems and implement solutions to the cause rather than the symptom, preventing ongoing occurrence and associated costs.
This training course explains the RCA process and how to use it effectively to achieve real benefits to organisations. Implementing expert RCA training can potentially save millions in costly organisational issues and avoid loss of reputation.

Chris Puschak has held leading roles in the delivery of projects including company mergers, outsourcing, major ICT replacement activities and business re-engineering. Since 2001 he has worked in the audit, risk and compliance fields within government business enterprises or as a consultant to government, and he is well respected for his insight, forthright nature, can do attitude, and consultative approach.
Chris is well known for his stakeholder relationship skills, which resulted in a significant improvement in regulator relationships for both Medibank and Airservices during his roles in the assurance and risk teams. He has held senior roles and provided strategic direction and insight to senior executives across the ACT and Commonwealth Governments and private industry for the past 20 years.
